Acne generally begins building with a proliferation of sebum from sebaceous glands beneath the skin. This protective but oily substance rises to the skin's top layer and exits through the pores to help lubricate the skin. This then results in blackheads and pimples in most people, but may give rise to cysts beneath the skin in serious cases. Manual manipulation of these blemishes can culminate in the spreading of the bacteria and increase the possibility of leaving an acne scar.

In some cases, what may appear to be an acne scar is not in reality a scar; rather, it might be only a somewhat inflamed or red spot where the acne lesion has run its course. These small red spots, also called macules, look very much like a scar but, fortunately, they normally disappear completely in about five to six months.

In other cases, although an acne sore has healed, there might be some discoloration in the skin's pigment, but this can often be misidentified as an acne scar. These marks will normally disappear in about 12 to 18 months, growing lighter as time passes, and are not truly scars.
